This is a brief, designed by RM (Real Men). String brief style, as pictured. The pouch is designed to fit narrowly, so there's only one place for the twig and berries to go: up and out. It gently squeezes the balls forward. The sizing is hard to describe : the front pouch seems like a large, but the butt area fits like a medium - probably intentionally.
